Faith in Action is sponsoring an Affordable Care Act information and enrollment workshop 9 a.m. to 4 p.m. Saturday at Emmanuel Lutheran Church, 1900 Baker St.

Attendees may pick up eligibility forms to be pre-screened and schedule an appointment with a certified health enrollment counselor.

Eligibility forms, due Wednesday, may be obtained and turned in at Emmanuel Lutheran Church, St. Joseph's Catholic Parish, 1515 Baker St., or at the offices of Noble, Longfellow or Jefferson elementary schools.

People without appointments may also attend the workshop. Basic documents are required to complete the enrollment process for each person in the household claimed in tax returns. These include a birth certificate or passport if the person was born in the United States or a citizenship certificate or green card for those born outside the U.S.

Other documents needed are a most recent tax return, proof of current income, social security cards, proof of residency, such as a utility bill, and a picture ID.

The workshop is sponsored by Faith in Action Kern County in partnership with the Community Health Initiative of Kern County.
